CIRRHOSIS OF LIVER

This is not a primary disease but the end result of diffuse liver injury due to many factors. These are hepatic changes initiated by cellular necrosis resulting into collapse, regeneration, fibrosis and altered intra-hepatic circulation.

Causative factors


1) A high carbohydrate diet with low protein and amino acid may contribute to liver insult. Also unsaturated fats increases the susceptibility to liver damage
2) Alcohol
3) Viral hepatitis
4) Infections
5) Alfa toxins – Aspergillus sp. found in groundnuts is hepatotoxic
6) Drugs – methotrexate (cancer & psoriasis drug, also used for rheumatoid arthritis) & other drugs
7) Metals – iron toxicity, metal toxicity
8) Cryptogenic – in some cases exact cause cannot be found

Signs & Symptoms


 In its early stages hepatic cirrhosis may produce no symptoms at all, but in advanced cases following symptoms are seen
1) Ascites – collection of fluid over abdomen. Abdomen appears distended, abdominal veins are usually seen
2) Oedema (swelling) on legs
3) Attacks of diarrhea or fever
4) Haematemesis ( blood in vomiting)
5) Spider like black formation can be seen on body
6) Jaundice accompanied by weight loss, no appetite

Investigation


1) Ultrasonography shows shrunken liver with irregular surface or enlarged liver
2) Liver function test – elevated bilirubin
3) Urine analysis – shows urobilinogen
4) Needle biopsy confirms the diagnosis

Therapeutic consideration


1) Stop alcohol, highly refined carbohydrates and unsaturated fat, salt
2) A nutritive, easily digestible high protein diet helps a lot. Best of all will be milk and vegetable proteins
3) Iron – because of defective absorption and digestion*
4) Vit-B12 –because of defective absorption and digestion*
5) Potassium – potassium loss is prominently due to diuretics given to reduce swelling *
6) If jaundice, then care should be taken as per given in jaundice

Ayurveda approach


According to ayurveda, this is a advanced type of PITTA disorder where more DHATUS are involved and BALA of patient is strongly reduced. Though it is said to be difficult to cure, it can be controlled completely with ayurvedic medicines and a very strict diet regime. Therefore, ayurvedic medicines are strongly recommended for this problem. Ayurveda is arguably the only science in this world to claim that this problem can be encountered completely.
